All plans include $0 preferred prescriptions through Curative Pharmacy or Publix.
Curative offers four plans:
EPO Value
Lowest premium. Requires use of a Curative Pass for $0 care with recommended providers.
EPO
Broader in-network provider access. No Curative Pass required. $0 care with in-network providers. No out-of-network coverage.
PPO
Same network provider access as EPO, but copays and deductibles for out-of-network access. Includes ClassPass membership.
PPO Max
Widest provider access. No deductibles for out-of-network care, but may be subject to balance billing. Includes ClassPass and an extended pharmacy network.

Choose one or more recommended provider and receive a unique Curative Pass for $0 out-of-pocket care for covered services

Introducing: The Curative Cash Card
We make it simple to get $0 care when and where you need it. Use it at in-network providers if they don’t accept your Member ID Card, or at any provider listed as Cash Card-approved on our search. Use it just like you would a payment card—only you don’t pay a thing.
